Growth Management Permit Applications / Forms

Starting November 1st, the Crystal River Growth Management Department will only accept electronic permit documents.

The Crystal River Growth Management Department is continually working to improve its efficiency for its contractors and citizens.  Going paperless will allow plans and supporting documentation to be sent electronically to multiple departments for review, resulting in an expedited permitting process.

How do I submit electronically?

Permit applications and plans will be emailed to development@crystalriverfl.org.

Requirements for submitting plans and supporting documentation are as follows:

  • All drawings and supporting documentation in the plan package must be in PDF format.
  • Each required document must be submitted as a single file.
  • Applications are still required to be filled out and submitted. Blank application forms for submittal can be found on the City of Crystal River’s website.
  • Documents that require a professional seal and signature must be digitally signed in accordance with professional requirements.

What can I expect after submittal?

Permit documents are reviewed, marked up, disapproved, or approved electronically.  Once the review of the documents is completed, applicants are notified by e-mail of any needed corrections or that the application has been approved.

HOW TO APPLY

Permit services is located at Crystal River City Hall, temporarily located in the Chamber of Commerce Building at 915 N. Suncoast Blvd, Crystal River, Florida, 34429. All forms are available online at our website or for pick up at our office. Permits can be submitted in person, by mail, or by emailing* to development@crystalriverfl.org. All documents must be in PDF format. Photos of documents will NOT be accepted. Incomplete submissions will be rejected, and you will be requested to resubmit when the submission is accurate and complete.  See WHAT TO SUBMIT below for details on submission requirements.

WHAT TO SUBMIT

REQUIREMENTS MAY VARY BASED ON PERMIT TYPE. All permit documents are required to be submitted with the permit application at the time of submittal. You must submit one (1) electronic copy (PDF) of all documents including the application and Contractor Registration. We will no longer be accepting paper copies. 

  • A completed permit application-must include the following:
    • Alternate Key: available at www.citruspa.org/_DNN/
    • Valuation of Completed Work (includes total value of work including materials and labor)
    • Proof of property ownership: available at www.citruspa.org/_DNN/
    • Notarized Signature
    • Please fill out the application in its entirety. Incomplete forms will not be accepted.
  • Owner Builder Form (if applicable)
  • Contractor Registration and supporting documentation (expired and/or choosing to not retain with the City):
    • Completed Application (must be signed and notarized)
    • State License
    • General Liability
    • Workers Compensation Form
    • Local Business Tax Receipt (if applicable)
    • Authorized Agent(s) Form (optional; must be signed, notarized)
  • Notice of Commencement
    • Must be on file prior to the first inspection.
    • If the Value of Construction is greater than $5,000.00, a certified copy of the recorded Notice of Commencement is required.
    • For Mechanical Permits: if the Value of Construction is greater than $15,000.00, a certified copy of the recorded Notice of Commencement is required.
  • Product Approval (if applicable)
    • Must use the City’s Product Approval form.
    • Must be signed and dated.
  • Project Plans & Construction Drawings, if applicable, may include but not be limited to:
    • Foundation Plans
    • Floor Plans
    • Floor-Roof Framing Plans
    • Wall Sections
    • Roof Plans
    • Exterior Elevations
    • Electrical Plans
    • Mechanical Plans
    • Plumbing Plans
    • Wind Design Data
    • Additional Details (as necessary)
  • Elevation Certificate-Must submit an NFIP Elevation Certificate for new construction or significantly improved/damaged buildings. Permit files must have official records indicating new buildings and substantial improvements to buildings in all identified Special Flood Hazard Areas (SFHAs) are elevated properly. Elevation information needs to show floodplain management ordinance compliance.
  • Energy Calculation-Must submit an energy or heat load calculation for all new buildings and substantial improvements to identify efficiency of heating and cooling in the building.

FEES

The permit fee schedule is posted at Crystal River in the Development Services Department and is available upon request. A breakdown of the permit fees is available upon request. Once the permit has been invoiced, you will receive an email at the address provided on the permit application detailing the permit fee amount. Fees can be paid in person, over the phone by calling the the permitting department, or by mailing a check to City Hall. Please include the permit number on the check.

PERMIT ISSUANCE

Permits must be paid for in full prior to issuance. Permits can be picked up picked up from our office Monday-Friday, 8:30am-4:30pm.

Permits are valid for 180 days from the date of filing unless such application has been pursued in good faith or a permit has been issued. Please see below for information on PERMIT EXTENSIONS or PERMIT REINSTATEMENTS.

INSPECTIONS

It is the responsibility of the permit holder to request the appropriate inspections required. Inspection requests can be made by phone but are preferred by email. The inspection request must include, at a minimum:

  • Permit Number
  • Address
  • Name of Contractor
  • Contact Name
  • Contact Phone Number
  • Type of Inspections

Inspections may not be made more than three (3) days in advance. Inspection request cutoff is 3:00pm for the next business day.

For roofing final inspections: a signed Inspection Affidavit must be submitted prior to the scheduling of the final inspection.

PERMIT EXTENSIONS

A permit extension must be applied for by submitting the 90-Day Permit Extension form and submitting it prior to the expiration of the permit. The extension form must be filled out in its entirety. Incomplete forms will not be accepted. The extension is only good for 90 days and must be approved by the Building Official. There is not a limit to the number of extensions an applicant can apply for. The processing fee for a permit extension is $50.00.  

EXPIRED PERMIT REINSTATEMENTS

An expired permit reinstatement request may be made if the permit has expired by submitting the Expired Permit Reinstatement Request form. The reinstatement request form must be filled out in its entirety. Incomplete forms will not be accepted. The request must be approved by the Building Official. The processing fee for a permit reinstatement is $100.00 if there is only a Final Inspection required. If the permit expired prior to any inspections, the processing fee is $120.00.

EMERGENCY REPAIR PERMIT PROCEDURE

Unexpected work may be required in an emergency. Please submit a permit application to Development Services Department via email at development@crystalriverfl.org and advise of the emergency repair. Please include the words “Emergency Permit” in the description of work that is to be performed.

PERMIT REVISIONS

If you would like to make changes to a permit application after it has been submitted OR after the permit has been issued, please request the change by submitting a Permit Revision Request form via email. The revision request form must be filled out in its entirety. Incomplete forms will not be accepted. You must submit one (1) electronic copy (PDF) of all documents. The request must be approved by the Building Official. The processing fee for a permit revision is $100.00.

PERMIT CANCELATION

If you would like to cancel a permit application, please submit a written request of cancelation on a letterhead detailing the permit number and site address. Make sure the document is dated and signed before returning.

CERTIFICATE OF OCCUPANCY

For a Certificate of Occupancy to be issued by the Building Official, all required certifications and documentation must be on file, all fees must be paid in full, and all inspections passed with approval.
